A computer-based cooperative system for the management of heart failure

This project is financed by the Provincia Autonoma di Trento and coordinated by the ITC-Irst in collaboration with the Universities of Pavia, Florence and Trento, the Fraunhofer IBMT and the Health Care Service Trust of the Province of Trento. The general aim of this project is to design, develop and evaluate a computer-based co-operative work framework for supporting guideline-based shared care delivery.

A web-based framework will act as a virtual environment to provide information management services directed to the different classes of users. This co-operative framework based on innovative knowledge management techniques will support the different people involved in the care process (i.e. hospital physicians and nurses, and general practitioners) to provide information, to give access to information according to their needs, and to exchange information among them.

The framework will be implemented and validated in a clinical setting for evaluating the effectiveness of the approach in managing heart failure patients.
